What you'll do

• Lead hospice admissions
• perform eligibility and comprehensive nursing assessments
• assess medical, physical, emotional, and social needs
• develop and coordinate individualized care plans
• educate patients and families
• facilitate informed consent and complete admission documentation
• provide emotional support
• initiate pain and symptom management
• maintain accurate EMR documentation
• coordinate with referral sources and interdisciplinary colleagues
• travel throughout the service area.
