What you'll do

• Oversee development, implementation, supervision, and evaluation of the Social Services Department
• identify residents’ medically related social-service needs and ensure services are provided in accordance with regulations
• complete or delegate social-service assessments and care-plan contributions
• oversee preadmission services
• coordinate discharge planning, community-based services, and follow-up care
• establish and maintain departmental quality-assurance procedures.
