The Afterhours Registered Nurse (RN) Care Manager is an onsite role based in Alpharetta, GA. This position supports the Gentiva Hospice team by providing compassionate, high-quality hospice care outside of standard business hours. This role exists to ensure seamless coverage and continuity of care for patients and families during nights, weekends, and on-call hours. The Care Manager provides clinical oversight, conducts assessments, and coordinates with the interdisciplinary team to maintain high standards of end-of-life care.

What you'll do

• Provide direct in-person patient care during on-call, weekend, and after-hours shifts.
• Conduct initial and ongoing assessments of physical, emotional, and psychosocial needs.
• Develop and revise the Hospice Plan of Care in collaboration with physicians and the IDG.
• Educate patients and families on disease progression and symptom management.
• Maintain accurate clinical documentation and support administrative compliance tasks.
