Primary Care Physician (PCP)

What you'll do

• Provide primary medical care to PACE participants
• conduct evaluations and manage acute and chronic conditions
• develop treatment plans
• participate in interdisciplinary team meetings
• coordinate care and referrals
• order and interpret diagnostic tests
• prescribe and manage medications
• provide clinical support to nurse practitioners and other staff
• maintain timely medical records
• participate in quality improvement and an on-call rotation as needed.
