Registered Nurse (RN) Case Manager

The Registered Nurse (RN) Case Manager role is onsite in Tulsa, OK, with a stated schedule of Monday–Friday, 8:00 a.m.–5:00 p.m. Compensation is not specified. What you'll do

• Develop patient plans of care with the interdisciplinary team, patient, family, and attending physician
• ensure indicated services are provided
• participate as a core interdisciplinary team member
• supervise home health aides, certified nurses aides, and homemakers
• report significant condition changes and update plans as needed
• coordinate care across settings
• manage pain and symptoms
• instruct patients and families in care techniques
• review the Medicare Hospice benefit and available services
• complete required documentation and participate in training and quality improvement activities.
