What you'll do
- Complete MDS assessments in accordance with RAI and CMS guidelines
- coordinate assessment timelines
- lead and participate in interdisciplinary care planning
- review documentation for accurate coding and PDPM reimbursement
- collaborate with nursing, therapy, social services, and dietary teams
- monitor quality measures
- educate nursing staff on documentation practices
- maintain accurate resident care plans
- support survey readiness and regulatory compliance.
