Community Outreach Specialist — Personal Care Services / Home Health

What you'll do

• Represent AbriCare at senior events, health fairs, food pantries, and community gatherings
• share information about services
• identify visibility opportunities through volunteering, sponsorships, and events
• build relationships with hospitals, social workers, AAAs, VA centers, senior programs, faith communities, housing authorities, and nonprofits
• coordinate outreach events
• distribute materials
• maintain outreach database records
• educate community members about Missouri Medicaid personal care services
• gather and share community feedback.
