What you'll do

• Assess patients during visits and chart observations
• perform vital signs, medication review, OASIS assessments, wound care, and IV therapy
• dress or redress wounds and assess healing
• educate patients and families on home health care
• coordinate with occupational, physical, and speech therapists, physicians, and others involved in care
• recommend devices or tools that may improve quality of life
• respond to patient concerns.
