Job Title/Position: Patient Care Coordinator. Reports to: Clinical Manager. JOB DESCRIPTION SUMMARY: The Patient Care Coordinator’s (PCC) primary responsibility is to provide administrative support for the Clinical Managers, clinical care staff and patients. The PCC will coordinate daily/weekly/monthly clinical reports, patient supplies and interagency/community referrals.
