What you'll do

• Provide skilled hospice nursing care and support to patients and families
• coordinate comprehensive care for a caseload
• assess and respond to patient, family, and caregiver needs in home and facility settings
• collaborate with physicians, facilities, and interdisciplinary team members
• complete required documentation and manage deadlines
