Service Coordinator, Home and Community-Based Services

What you'll do

• Coordinate admissions, assessments, intakes, and onboarding
• develop and maintain Individual Service Plans and client records
• conduct monthly face-to-face visits
• collaborate with House Leads, care teams, and community partners
• assist with referrals
• follow up on incident reports and quality recommendations
• maintain documentation and compliance.
