What you'll do

• Monitor federal and state regulations and recommend policy or procedure changes
• implement processes to identify residents' medically related social and emotional needs
• participate in community planning, discharge planning, social care plans, and resident assessments
• consult with staff and community agencies
• develop correction plans for social service deficiencies
• maintain reference materials for compliance
• conduct and document social services evaluations
• communicate with caseworkers
• provide social work consultation to residents, families, and staff.

Why this role
