What you'll do

• Assess patients and families for hospice needs
• provide nursing care and develop, initiate, and revise plans of care
• administer prescribed medications and treatments
• counsel and educate patients and caregivers
• document clinical notes and communicate changes to physicians
• coordinate assigned caseloads and attend interdisciplinary meetings
• participate in on-call duties
• supervise hospice aides when required.
