What you'll do

• Provide nutrition assessments for admissions, quarterly and annual reviews, tube feedings, and significant changes.
• Create and update care plans, complete MDS documentation, and conduct nutrition-focused physical exams.
• Collaborate with the interdisciplinary team, participate in QAPI meetings, and perform kitchen inspections.
