What you'll do

• Conduct psychosocial assessments of hospice patients and families

• connect families with community resources, financial assistance, and support services
• assist with advance directives and healthcare proxies
• participate in team meetings
• provide bereavement follow-up
• maintain timely clinical documentation in compliance with hospice regulations.

Why this role
