What you'll do

• Coordinate with the Special Diabetes Program for Indians team and other health programs
• support patient care through communications and appointment scheduling
• take and distribute meeting minutes
• maintain inventory of program supplies and incentives
• maintain quality measures
• collect and analyze event data
• support audits, grant reporting, classes, and events
• order food and supplies
• create or update health education materials
• collaborate with Care Coordinators
• assist with diabetes prior authorizations and 340B pharmacy calls
• submit accurate reports.
