Compensation is not specified. The Registered Nurse — In-Home Health Assessments is a field-based role serving members across Massachusetts. Activate Care works to improve health equity and outcomes through care coordination and community-based support for people with health-related social needs. The nurse conducts in-person assessments, documents findings, and promptly hands them off to a health plan’s care management team. The role focuses on accurate assessment and documentation—not care-plan development or ongoing case management—and supports timely, compliant care coordination.

What you'll do

• Travel to members’ homes or community settings to conduct required physical, cognitive, and functional assessments
• document findings in GuidingCare and send SOAP notes to the assigned care manager
• submit complete documentation within 48 hours
• resolve audit findings within two business days
• follow field-safety guidance, escalate clinical or safety concerns, comply with mandated reporting laws, and follow HIPAA and NCQA requirements.
