What you'll do

• Assess and diagnose behavioral health clients
• develop treatment plans and support goal achievement
• evaluate crisis situations and apply appropriate interventions
• participate in integrated-care meetings
• help identify at-risk patients and develop prevention plans
• support primary care care-management processes
• provide assessment, consultation, and brief intervention
• teach patients, families, and staff care and prevention techniques
• monitor the site program and recommend improvements.

Why this role
