What you'll do

• Assess patients and families initially and on an ongoing basis
• develop, implement, and update individualized care plans
• provide skilled nursing, pain and symptom management, and medication administration
• educate patients and caregivers
• coordinate with physicians, social workers, chaplains, aides, and volunteers
• document in electronic medical records
• participate in interdisciplinary team meetings.
