This role supports primary care providers with clinical documentation. The purpose of this Medical Scribe role is to assist primary care providers with clinical documentation, allowing them to focus on patient care. The scribe will observe and document patient encounters in real-time, becoming an expert in the value-based care model and chronic condition documentation, including coding. This role is ideal for individuals seeking clinical experience before medical school or pursuing careers in health informatics.

What you'll do

• Documenting Patient Encounters (80%): Observe patient visits, document encounters in a structured note, assign CPT and ICD-10 codes, prepare After Visit Summaries, consult with provider for accurate documentation.
• Clinical Documentation Improvement (10%): Request and review medical records, leverage population health tools, prepare for and support Daily Huddles and Clinical Documentation Reviews, consult with provider on documentation opportunities.
• Administrative support (10%): Place orders and referrals, address tasks, support care team with documentation-related responsibilities.

What we're looking for

Education

High School Diploma or GED

Experience

Knowledge of medical terminology and common medications, either from a pre-medical degree or prior clinical experience.
