What you'll do

• Respond to patient calls with onsite nursing or protocol-based triage and route clinical inquiries
• return calls and communicate results as directed
• review schedules and gather records for visits
• arrange interpretation services
• participate in daily huddles
• support care transitions and ancillary service access
• maintain the Care Coordination Desktop
• assist with medication refills and prior authorizations
• prepare forms and referrals
• maintain patient records and follow safety and compliance practices.
