Case Manager

The Case Manager role is onsite in Salt Lake City, UT. Pay is $19 per hour. Odyssey House is a nonprofit mental health and addiction treatment organization serving clients and the community. The case manager assesses client needs, coordinates care with clinical staff and community resources, documents services, and supports clients’ progress toward treatment and personal goals.

What you'll do

• Interview and assess client needs and resources
• coordinate with licensed therapists on treatment plans
• monitor outcomes and client compliance
• maintain records and complete documentation within 24 hours
• verify Medicaid funding eligibility and status
• arrange appointments and connect clients with community resources
• communicate with collateral supports
• document incidents and prepare reports. Accompanying clients on recreational outings is an opportunity.

Why this role

Full-time schedule Monday–Friday, either 8 a.m.–4 p.m. or 9 a.m.–5 p.m. A related bachelor’s degree, two years of related employment, and/or program graduate status recommended by staff is preferred. Variable shifts may be needed.
