Client Care Manager (Senior Care Coordinator)

What you'll do

• Build relationships with clients and families
• conduct in-home visits, wellness checks, and reassessments
• develop and update care plans
• identify changes in client needs
• conduct home-safety reviews, fall-risk assessments, and cognitive screenings within the scope of non-medical services
• coordinate care conferences
• address concerns
• coach caregivers
• collaborate with office teams
• and document visits and follow-up.
