What you'll do
- Maintain the care-conference calendar and conduct interdisciplinary meetings as needed
- assess residents through record review, interviews, and direct assessment
- complete MDS, RAPS, resident summaries, and care plans
- ensure timely RAI completion and transmission
- lead managed-care case management as applicable
- participate in CMI, billing, and Medicare meetings as assigned
- communicate PDPM recommendations
- review state validation reports and correct as needed
- review Medicare coverage appropriateness
- obtain monthly Quality Indicator Reports.
