Registered Nurse, PACE Eligibility Assessments

The Registered Nurse, PACE Eligibility Assessments role is onsite in Stockton, CA. Pay is $40–$46.24 per hour. The role supports participant enrollment and state level-of-care review processes through nursing assessments in prospective participants’ homes. The nurse evaluates clinical, functional, and home-safety needs to help determine PACE eligibility, then documents findings for state review. The role contributes to timely eligibility decisions by coordinating with the interdisciplinary and outreach and enrollment teams and tracking annual reviews.

What you'll do

• Conduct initial nursing, functional, and home-safety assessments in prospective participants’ homes
• prepare and submit home-visit documentation to the California State Level-of-Care nurse
• respond to review questions
• track and oversee annual state level-of-care reviews
• collaborate with the interdisciplinary team and outreach and enrollment team
• protect confidential information.
