What you'll do

• Visit foster homes at least monthly
• build relationships with children, foster families, conservators, and treatment teams
• support foster parents with licensing standards
• coordinate educational advocacy and attend ARD meetings
• attend judicial hearings
• document visits and report suspected abuse or neglect
• develop and monitor service plans
• follow up on health care needs
• coordinate services across healthcare, education, and behavioral health.
