What you'll do

• Complete comprehensive assessments and coordinate individualized care plans
• lead care conferences
• review nursing reports and clinical trends and coordinate interventions
• participate in wound rounds and monitor skin integrity
• coordinate physician and nurse practitioner rounds
• support fall prevention, nutrition monitoring, infection prevention, and resident safety plans
• educate and advocate for residents and families
• coordinate admissions, transfers, and discharges
• collaborate with the MDS team
• maintain clinical documentation and support quality improvement and compliance.
