Hospice Certified Nursing Assistant (CNA)

Job overview

The Hospice Certified Nursing Assistant (CNA) provides onsite care in patients’ homes across Kewanee, Rock Island, and surrounding areas, with local travel for visits. The schedule is Monday through Friday in eight-hour shifts. Pay is $19–$23 per hour. Transitions Hospice provides 24/7 support for patients and families’ physical, emotional, and spiritual needs. The CNA provides dignified personal care and a dependable presence for patients and families. The role monitors and records vital signs, reports changes to the care team, communicates confidentially, and documents care in keeping with organizational standards and the hospice philosophy.
