Virtual Care Access Concierge – Registered Nurse (RN)

What you'll do

• Serve as a patient liaison for scheduling
• advocate for patients and families
• implement discharge plans and coordinate reminders and follow-up visits
• provide warm handoffs
• liaise with ambulatory clinics, free-standing emergency departments, and virtual services
• coordinate care transitions
• reinforce after-visit summaries
• identify patient problems and use resources
• document orders, care plans, and interactions in the electronic medical record.

Why this role
