What you'll do

• Provide social services support to residents and families
• assess psychosocial needs and assist with individualized care plans
• provide emotional support, counseling, and crisis intervention as needed
• participate in interdisciplinary care planning
• support adjustment to the facility and changes in health or living circumstances
• coordinate discharge planning and transitions to appropriate care
• connect residents and families with community, financial, and healthcare resources
• advocate for resident rights, dignity, privacy, independence, and quality of life
• assist with advance directives, guardianship concerns, admissions, transfers, and discharges
• respond to concerns
• maintain accurate documentation and regulatory compliance
• participate in meetings and quality assurance activities.
