What you'll do

• Provide medication management, consultation, and education to participants and caregivers
• co-manage medication therapy with primary care providers and interdisciplinary teams
• lead medication stewardship, including deprescribing and reducing medication burden
• conduct medication reviews
• use participant and population data to identify medication-related risks
• provide drug information and clinical education
• collaborate on medication access and therapeutic substitutions
• document clinical activities and interventions.
