Physical Therapist (PT) - Travel Contract

Provide physical therapy assessments and treatment for neuromuscular, neurological, and orthopedic conditions in home settings. Prepare and revise Plans of Care, document progress notes, educate patients and families, and coordinate with interdisciplinary teams. Order and adapt durable medical equipment and instruct patients in safe use of assistive devices.
