What you'll do

• Conduct patient assessments and develop or update individualized care plans
• provide skilled nursing, including wound care, medication administration, and patient education
• coordinate and supervise home health aides and other care professionals
• communicate patient progress and treatment plans
• maintain regulatory and agency documentation
• evaluate outcomes and participate in quality improvement and continuing education.
