Per Diem Residential Counselor

Job overview

The Per Diem Residential Counselor role is in person at available residential locations, including Oakdale, NY; compensation is $20–$24 per hour. Options for Community Living works to help vulnerable Long Island families and individuals live healthier, more stable, and productive lives. The counselor provides per-diem support to residents with mental health diagnoses, helping them with daily living, service-plan objectives, medication documentation, and case-management follow-up. The role also supports a safe, professionally managed community residence and responds to behavioral, crisis, and medical needs according to procedures.

What you'll do

• Participate in planning and evaluating individual service plans and monitor objectives
• train and assist residents with activities of daily living
• document services and medication
• provide case management and follow-up
• provide counseling, behavioral intervention, and crisis management while following emergency procedures
• assist with household and residence management
• transport residents as needed
• notify supervisors of medical or psychiatric emergencies
• complete overnight checklists and logs when working overnight
• maintain training requirements.
