What you'll do

• Conduct in-home nursing assessments and head-to-toe evaluations, including medication and diagnosis reviews
• develop, review, and update Plans of Care
• supervise and orient home health aides
• provide clinical guidance
• monitor patient condition and communicate concerns
• collaborate with patients, families, caregivers, aides, and office staff
• complete timely clinical documentation
• travel to patient homes and community settings in assigned service areas.
