What you'll do

• Review resident referrals and respond to home care requests
• coordinate assessments and care plans with physicians and other healthcare professionals
• promote home health and wellness programs
• monitor resident changes and update care plans and service contracts
• contact physicians and family members as needed
• support resident satisfaction
• participate in hiring, supervision, training, performance management, and corrective action.
