What you'll do

• Engage and retain young people in services
• conduct intakes, assessments, service planning, and case coordination
• connect clients with community resources
• monitor progress toward goals
• provide skill-building, coaching, and mentoring
• conduct regular apartment visits
• maintain communication with young people and families
• participate in project evaluation and reporting
• and participate in the emergency services on-call rotation. A valid driver’s license and reliable transportation are required.
