Resource Coordinator

What you'll do

• Assess participant needs and link them to appropriate services
• maintain regular contact with participants and service providers
• develop and review individualized, strength-based goal plans
• conduct home visits and accompany participants to appointments as appropriate
• provide information and advocacy about benefits, treatment, and supports
• coordinate outreach with the team and community agencies
• and maintain service records and reports.
