The Registered Nurse – Clinical Case Consultant, ECM role is hybrid in Tulare, CA, with remote, office-based, and field-based work. It is part-time, scheduled Monday through Friday from 8:00 AM to 1:00 PM, with flexibility; compensation is $45 per hour. Pacific Health Group’s Enhanced Care Management program supports high-risk Medi-Cal members with complex medical, behavioral, and social needs. The RN serves as a clinical resource for interdisciplinary care teams, providing consultation, field oversight, and LVN supervision. The role supports coordinated, person-centered care, quality improvement, and compliance with CalAIM ECM requirements while helping reduce avoidable utilization and improve member outcomes.

What you'll do

• Provide clinical consultation to care teams
• review complex cases and recommend care planning and risk interventions
• support individualized care plans and ECM requirements
• conduct field visits, shadow staff, coach teams, and review field documentation
• supervise and mentor LVNs
• coordinate with providers, hospitals, health plans, and community organizations
• support care transitions
• conduct quality reviews
• deliver staff training
• contribute to quality improvement, reporting, and program development.
