What you'll do

• Provide strategic leadership and operational oversight for case management, discharge planning, social work, and access control
• develop standardized care-coordination workflows
• collaborate with physicians, nursing leadership, and senior administration on patient flow, length of stay, and outcomes
• ensure timely post-acute pre-certification
• identify process improvements and implement clinical pathways with providers
• manage budget, staffing, and performance
• oversee KPI data and reporting
• participate in interdisciplinary meetings and case-management responsibilities
• maintain accountability for acute inpatient social work.
