The Hospice Music Therapist - Per Diem role is onsite in San Jose, CA, with visits to patients and families; the posting states pay of $35 to $38 per hour. The therapist supports ANX Hospice Care’s interdisciplinary hospice team. The role provides medically prescribed music therapy as part of patients’ hospice plans of care. Through assessment, individualized interventions, ongoing evaluation, documentation, and communication with the interdisciplinary team, the therapist supports patients’ and families’ physical, emotional, cognitive, spiritual, and social needs.

What you'll do

• Assess patients and families
• develop individualized music therapy goals and plans
• deliver interventions consistent with the interdisciplinary plan of care and AMTA standards
• evaluate and document services
• communicate observations and supportive music guidance to the interdisciplinary team, caregivers, and families
• provide delegated in-services
• determine when services should end.

Why this role
