What you'll do

• Assists in resident assessment upon admission and on an ongoing basis.
• Communicates significant changes in condition to the interdisciplinary team.
• Documents all assessments and interventions accurately.
• Participates in the care planning process and updates plans as needed.
• Implements nursing interventions, including medication administration and treatments.
• Notifies physicians of changes using the SBARR tool.
• Monitors resident status for behavioral changes.
• Answers call lights and assists with activities of daily living.
