Care Coordinator / Case Manager

The Care Coordinator / Case Manager role is onsite in San Antonio, TX. Pay is $20 per hour. The position supports Graceland Psychiatry’s Dual Diagnosis IOP PHP Treatment Team, which provides client-centered care. The coordinator helps clients navigate treatment by maintaining communication, coordinating transportation and resources, supporting intake, and documenting case-management activity. The role connects clients, families, community resources, and the multidisciplinary treatment team to address barriers and support continuity of care.

What you'll do

• Communicate with clients about treatment needs and appointments
• identify barriers to participation
• assist with forms and referrals
• connect clients with housing, transportation, employment, financial, medical, and recovery resources
• support intake and admission
• maintain patient profiles in the EHR
• coordinate rides
• share relevant updates with the treatment team
• document case-management activities and follow confidentiality requirements.
