Care Management Professional

Job overview

The Care Management Professional is an onsite role based in Waynesville, NC, with a pay range of $25–$32.10 per hour. This position supports the Appalachian Community Services team in delivering comprehensive behavioral health and developmental disability services to the local community. This role exists to proactively engage high-risk members and coordinate their whole-person health needs. The Care Manager assesses member requirements, develops individualized service plans, and facilitates communication between various healthcare providers to ensure seamless, effective care delivery.

What you'll do

• Monitor members for clinical changes, evaluate findings to develop Individualized Service Plans, coordinate care with primary providers, facilitate community resource referrals, and maintain accurate clinical documentation.
