Licensed Practical Nursing Associate

Job overview

Provides basic nursing care by assisting in the assessment of the resident on admission and on an ongoing basis. Remains knowledgeable about and communicates to the appropriate interdisciplinary team (IDT) member any significant change in condition. Accurately documents all assessments. Seeks guidance if unsure of any assessment findings. Participates in the care planning process for the resident. This role is essential for implementing nursing interventions and ensuring the care plan is effectively delivered. The associate uses critical thinking to manage resident changes in condition and collaborates with the IDT for problem resolution, contributing to the overall health and well-being of residents.

What you'll do

• Assists in resident assessments, documents findings, and communicates changes in condition to the IDT.
• Participates in and updates the care plan.
• Implements nursing interventions, administers medications and treatments safely.
• Notifies physicians using SBARR.
• Manages crisis situations professionally.
• Monitors resident care and uses QAPI for problem resolution.
• Answers call lights and assists with ADLs.
• Completes continuing education.
• Keeps current with nursing practices.
