Activities Coordinator

Job overview

The Activities Coordinator role is onsite in Warren, OH; compensation starts at $15.50 per hour and increases to $16.50 per hour after 90 days. Buckeye PACE is part of One Senior Care, whose team works to keep participants safe, healthy, connected, and at home. The coordinator plans and leads activities that support participants’ physical, mental, and social well-being. The role encourages participation, documents activity plans, and collaborates with the Interdisciplinary Team to support each participant’s care plan and daily connection.

What you'll do

• Plan and lead exercise, games, crafts, music, and special events
• encourage participants to join activities suited to their interests and abilities
• create monthly activity schedules and maintain participant chart documentation
• work with the Interdisciplinary Team to support care plans
• share updates about participant needs and preferences
• assist with the annual activities budget and supplies
• participate in quality improvement initiatives and ongoing training.
