The CNA provides personal care and daily-living assistance in accordance with patient care plans, working under the direction of a Case Manager RN. The role supports patient comfort, safety, and continuity of care through mobility assistance, nutrition support, and accurate electronic records.

What you'll do

• Provide bathing, dressing, grooming, toileting, and skin care
• review patient care plans
• assist with lifting and transfers, range of motion, positioning, exercise, and ambulation
• assist with dining and nutrition
• maintain electronic medical records
• work under the direction of a Case Manager RN.
