Case Manager — Family Services

What you'll do

• Manage an assigned caseload and coordinate in-home services
• collaborate with DCYF and families on referrals and treatment plans
• document assessments and service delivery
• report progress to courts as requested
• provide parent education, coaching, and supervised visitation
• assess risk and consult the clinical supervisor
• transport clients
• participate in an on-call rotation.
