The Registered Nurse (RN) — Home Health & Hospice role involves onsite patient visits in Onalaska and Coldspring, TX, including homes, assisted living, and hospice settings. Compensation is $38–$60 per hour. The RN joins the employer’s Home Health and Hospice team to provide skilled nursing care and support patient safety, comfort, and clinical outcomes. The RN assesses patients, develops care plans with physicians and families, administers ordered treatments, monitors changes in condition, and coordinates with interdisciplinary care teams. The role also includes patient and family education, timely electronic documentation, compassionate end-of-life support, and participation in an on-call rotation as assigned.

What you'll do

• Conduct initial and ongoing patient assessments
• develop individualized care plans with physicians, patients, and families
• administer medications, IV therapies, wound care, and other ordered procedures
• monitor and report patient progress and changes
• educate patients and families
• coordinate with interdisciplinary teams
• document in the EMR
• participate in the on-call rotation as assigned
• comply with applicable policies and regulations
• provide end-of-life care and emotional support.
