The role supports recovery and long-term wellness in collaboration with a multidisciplinary team.

What you'll do

• Conduct biopsychosocial assessments and ASAM level-of-care determinations
• develop and implement individualized treatment plans
• provide individual counseling and facilitate IOP and outpatient groups
• coordinate referrals, discharge planning, and aftercare
• complete progress notes and maintain CARF, Maryland COMAR, and payer-compliant records
• conduct treatment plan reviews and ASAM reassessments
• provide crisis intervention and risk assessment when needed
• participate in supervision, team meetings, case conferences, training, and quality improvement.
